The result of
ket{A^B}
Looks like the first pic here:

Questions:
Why doesn't it look like the second? That's what I would expect, since I have not defined the command as
left|#1rightrangle.How do I get the result as on the second pic? (always standard size)

That's odd and are you sure you have not loaded thebraketpackage (or something of that sort)? Could you please provide us with a full document that produces that output?
